When to use this template
Use this as a manual note-taking outline for an interview. Prepare role-related criteria and questions with your hiring team before the conversation. The template organises notes; it does not assess candidates, rank people or make hiring decisions.
Make it useful for your meeting
1. Agree the criteria before the interview
Record the role, interview stage and the skills this conversation should explore. Use the questions and evaluation process agreed by your hiring team. Keep the notes focused on information relevant to the role.
2. Record examples accurately
Write the question and the candidate's example in separate fields. Identify quotations only when you have captured the wording accurately. Distinguish an observation from your interpretation.
3. Keep unanswered questions visible
Record where the conversation did not provide enough evidence. Do not treat missing information as a confirmed weakness. Assign a follow-up question to the appropriate interviewer or coordinator.
4. Review and store the record appropriately
Check that notes relate to the agreed criteria and remove unnecessary personal details. Follow your organisation's hiring, access and retention process when you download or share the file.

Common questions
Does this tool evaluate the candidate?
No. It is an editable document with prompts for role-related evidence and gaps. It does not score, rank, screen or recommend candidates. The hiring team applies its own agreed review process.
Can I adapt the questions for another role?
Yes. Replace the criteria and prompts with those agreed for that role and interview stage. Keep the questions relevant to the work and record answers accurately.
